In this role you will evaluate patients, create and implement care plans, and track progress to maintain the highest standards of care. You will work closely with our Clinical Director and Clinical Staff, providing support both remotely and in the field. Responsibilities Provide, coordinate, and oversee skilled intermittent care in patients’ homes, including instruction and education of patients and/or caregivers. Regularly evaluate and re‑evaluate patient status. Administer treatments, monitor conditions, and report any changes to Physician, Case Manager, or Director of Nursing as needed. Initiate, develop, implement, and revise patients’ POC. Address all problems identified in the POC. Provide educational information to patients/families/caregivers regarding deficits or needs. Communicate with other disciplines involved as warranted. Maintain accurate clinical records and submit documentation per company policy using a company‑provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements Current PT license to practice in California (required). At least 1 year of experience in Home Health or Hospice care (required). Current California driver’s license. Able to travel within a 15‑25 mile radius on a regular basis.
