Occupational Therapist (OT) – Full-Time
Lock Haven Rehabilitation & Senior Center | Lock Haven, PA

Lock Haven Rehabilitation & Senior Center is seeking a compassionate and motivated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team. This is an excellent opportunity to work in a collaborative Skilled Nursing and Long-Term Care environment dedicated to helping residents maximize their independence and quality of life.

Position Details

• Full-Time

• In-House Rehabilitation Program

• Skilled Nursing Facility (SNF) / Long-Term Care (LTC)

• Collaborative interdisciplinary team

Qualifications

• Current Pennsylvania Occupational Therapist license or eligibility for licensure

• Strong clinical, communication, and documentation skills

• Commitment to patient-centered care

• Ability to work effectively within a multidisciplinary team

• SNF/LTC experience preferred, but not required

Technology

• PointClickCare (PCC)

Responsibilities

• Evaluate residents and develop individualized treatment plans

• Provide skilled occupational therapy services focused on improving independence with daily living activities

• Collaborate with physicians, nursing staff, and rehabilitation team members

• Complete timely and accurate documentation

• Educate residents, families, and caregivers on treatment goals and progress

• Ensure compliance with facility, state, and federal regulations
