Job Duties: (To be performed with or without accommodation as based on ADArequirements) Communicate daily with hospital liaisons to receive number of transports for each day, obtain individuals name and destination location. Assure individual, needing the transport, has collected all personal belongings and discharge paperwork information for follow-up. From hospital personnel and/or GCC Liaison, collect necessary hospital discharge records and other necessary paperwork (such as additional prescriptions) to upload into the Center's Electronic Healthcare Record (SmartCare) by the end of the day or by 8:30am on the next business day Work with supervisor of record, Director of Nursing for transport requests from other local or out of region hospitals Will regularly interface with Gulf Coast Center staff as well as other community agencies when applicable Participate in Random Moment Time Study if required As directed by supervisor, assist various MH Adult programs with clerical duties as assigned prior to performing transport duties. Work closely with assigned clinic program assistant to complete assigned general clerical duties Serve as back-up to front desk operations and/or central Call Center as needed and if available Participate in weekly huddles with other members of work group Will be responsible and accountablefor the adherence to and promotion of the agency compliance policy and willrequire communication with various departments to foster, develop and continueour community partnerships as it relates to the position. Treat all individuals with the utmostcourtesy, decency and respect. Provide responsive, and efficientservices. Interact with staff, consumers, localentities and advocacy groups to provide a service that meets local needs. Provide highest level of CustomerService for both internal and external customers. Required to report personnel leave;flex time and emergency leave to assigned supervisor. Strict adherence to the professional,ethical, and legal requirements set forth in GCC's Business Code of Conduct aswell as the Corporate Compliance Plan. Consistent adherence to the Center'sDress Code. Consistent adherence to Center safetyguidelines and expectations. May be required to travel withinBrazoria and Galveston Counties. Out of region travel may be requiredto attend trainings. Must maintain a clear driving recordand driver's license in accordance with our Driver Policy. Requires current Texas Drive r'sLicense, reliable transportation and must maintain current auto liabilityinsurance. N/A Supervision Receivesdirect supervision fromManager of Administrative Support Services Required Education Requires a high school diploma or equivalent. Preferred Education/Licensure/Certifications N/A Required Licensure/Certifications N/A Required Experience Experience working with the public/community is required. Preferred Experience Two years paid experience in the behavioral health field is preferred. Experience working with social service agencies in both Galveston and Brazoria Counties is preferred. Self motivated with good time management skills. Good interpersonal skills. General knowledge of local social service network. Ability to establish cooperative relationships with individuals with mental health diagnosis. Mature problem solving approach to daily work. Ability toexpress self clearly and effectively, orally and in writing. Requiresability to work within a team approach. Requiresproficiency with computer skills, programs and email. Working Conditions Work will perfe formed in the Office setting. May requirevaried work hours depending on need of clients. Out ofregion travel may be required to attend trainings. May requiretravel to various locations within Brazoria and Galveston Counties. Requirescurrent Texas Driver's License, reliable transportation safe and current withall state requirements, and liability insurance. Special Projects Assistsupervisor when requested to complete special projects and complete allassignments within a time frame negotiated with the supervisor. Completion ofother duties as assigned by supervisor as it relates to the program andposition. Job Related Physical/Mental Requirements & Environmental Conditions Physical Abilityto transport, adjust and/or move items up to 55 lbs. Abilityto ascend/descend stairs. Abilityto remain in a stationary position for long periods of time. Abilityto operate office equipment. Abilityto push/pull, crawl/squat as it relates to PMAB requirements; may be requiredtoperform PMAB restraints as it relates to program requirements. Abilityto detect and writeeffectively. Abilityto hear (with or without aid). Abilityto have visual acuity to operate a motor vehicle and observe objects withinwork setting. Mental Abilityto understand, communicate and comprehend accurate information with othersinEnglish. Abilityto reason and analyze effectively. Abilityto solve practical problems as it relates to position. Requiredto pass and maintain all Center mandated trainings. Work Site/Environmental Conditions Abilityto work alone. Abilityto work closely with others. Abilityto work closely with clients and/or family members with poor hygiene. Abilityto worklong and/or irregular hours. Maybe subjected to hostile environment. Maybe subjected to high crime environment. Potentialexposure to tobacco products. Maybe subjected to both inside and outside environmental conditions. Subjectto standard noise levels within working environment. Travelby motor vehicle, van, bus or plane may be required.
