Hospice Registered Nurse (RN) Case Manager | Full-time | Excell Home Care & Hospice OKC

Job Summary RN sign-on bonus $6000.00. Excell Home Care & Hospice is seeking a registered nurse to conduct initial visits for hospice referrals in acute care facilities or at home, identify patient needs, plan, organize, and direct hospice care with emphasis on community health education. Duties & Responsibilities Patient Care: Complete an initial, comprehensive and ongoing assessment of patients and families to determine hospice needs. Provide a full physical assessment and history of current and prior illnesses. Utilize the nursing process to plan, implement, and evaluate care. Write and initiate plans of care, and regularly re-evaluate and revise them as needed. Use health assessment data to determine nursing diagnoses and develop goals. Include patients and families in the planning process, initiating appropriate preventive and rehabilitative nursing actions. Administer medications and treatments as prescribed by the physician in the plan of care. Counsel patients and families on meeting nursing and related needs. Provide health‑care instructions per assessment and plan. Assist patients with activities of daily living and facilitate efforts toward self‑sufficiency and comfort care. Act as case manager, coordinating patient care for assigned caseloads. Communications: Complete, maintain, and submit accurate clinical notes regarding patient condition and care. Record pain/symptom management changes and outcomes. Communicate with physicians about patient needs and obtain orders as required. Coordinate with community health professionals and the interdisciplinary team. Teach self‑care techniques to patients and families. Maintain a safe environment and facilitate continuity of care. Attend interdisciplinary group meetings. Additional Duties: Participate in on‑call duties per policy. Ensure arrangements for necessary equipment and services. Supervise and delegate responsibilities to ancillary personnel. Engage in personal growth through continuing education and in‑service training. Participate in quality assessment and performance improvement activities. Job Requirements Graduate of a National League for Nursing accredited school of nursing. Minimum of two years of nursing experience, preferably in public health, home care, or hospice. Current licensure to practice professional nursing in Oklahoma. Excellent observation, problem‑solving, verbal and written communication skills. Ability to organize and prioritize workload independently. Physical capability to walk, stand, lift, transfer patients, lift supplies, and perform necessary reaching, bending, and stooping. Visual and hearing acuity sufficient for nursing tasks.
