Physical Therapist - Travel Contract

Minimum Requirements

Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active Indiana Physical Therapy license required to start the assignment.
BLS (Basic Life Support) certification from AHA or ARC may be required.
Home health experience required. Experience with resumption of care, start of care, and discharges preferred.
Experience using electronic documentation systems required. Specifically, experience with HCHB (Homecare Homebase) is required. Experience with eMAR systems is required.
Ability to oversee or coordinate with licensed practical nurses and other home health staff.
Strong communication skills preferred.
Must meet client-specific requirements for travel clinicians, including coverage of Marion and Hamilton counties.
Minimum education requirement: Bachelor‘s degree.

Assignment Details

Facility Type: Home Health.
Shift Name: Days.
Shift Details: Monday–Friday, 0800–1600.
Weekly Hour Guarantee: 36 hours per week.
Coverage Areas: Marion and Hamilton counties; travel between patient homes required. Mileage reimbursed between visits from first patient to last patient.
On-call/Weekend Requirement: None.
Productivity Expectations: 30 points.
Dress Code: Any solid color scrubs.
