BrightStar Care of Plymouth is a small but growing home care agency, looking to employ a Part-Time Physical Therapist (PT) to provide physical therapy treatment for our clients. This position does require traveling within our territory.2-3 days, no required rotation, no evenings and no weekends! Make your own schedule.Paid per Visit: $125 for initial visits, $85 for subsequent visitsPaid Drive Time & Travel ReimbursementFlexible Scheduling (make your own schedule)Weekly PayWork LaptopService Areas: Plymouth, Wayzata, Minnetonka, St. Michael, Rogers, Rockford, Buffalo, Delano and Maple Plain.Physical Therapist Requirements:Bachelor 's Degree or more in Physical Therapy from an accredited professional PT education programActive License on the Minnesota Board of Physical TherapyCurrent CPR CertificationAbility to travel up to 35 miles as neededExperience working with geriatric population, preferredPhysical Therapist Essential Duties:Provide physical therapy services to clients according to a physician's written orders. Perform direct physical therapy treatment.Assess and evaluate therapeutic/rehabilitative/functional status.Develop a plan of care and treatment making sure plan of care is consistent with assessment.Initiate physical therapy program and instructs other personnel and/or family members and caregivers in specific phases of physical therapy with which they may assist with the client care.Participate in discharge planning, as appropriate.Report client's response to treatment or changes in condition to the Director of Nursing and physician.Advance your career and join a compassionate and dedicated care team.
