Description

• Conduct comprehensive assessments of patients in inpatient and outpatient environments, devising customized treatment plans tailored to their needs
• Implement therapy interventions that emphasize mobility, strength, coordination, and day-to-day functionality
• Collaborate closely with a multidisciplinary teamnurses, physicians, case managers, and other rehabilitation specialiststo optimize patient outcomes
• Guide patients and their families through the therapeutic journey, focusing on their goals, discharge plans, and at-home exercise routines
• Maintain meticulous documentation of evaluations, therapies administered, and patient progress per facility protocols and regulatory standards
• Engage in collaborative meetings and care rounds to guarantee cohesive and comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualifications from an accredited institution

Certifications

• State licensure as a Rehabilitation Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological rehabilitation, geriatrics) are beneficial, although not mandatory

Skills

• Prior patient care experience is highly preferred
