Traveling RN Care Manager - Home Visits and Telemedicine

Your Health in Georgia is seeking a Registered Nurse to join our growing primary care team as a Visiting Nurse Case Manager. You will travel to patients' homes and facilities, coordinate with the care team, and initiate telemedicine visits as needed. The role requires reliable transportation, a valid RN license, and the ability to work 8-hour shifts Monday–Friday (8am–5pm).
