Licensed Rehabilitation Therapist - Skilled Nursing Facility

Location: Hawaii

Assignment Duration: This is a 13-week contract position.

Schedule: Variable schedule - 5 days per week, including potentially one weekend day.

Job Description: The Licensed Rehabilitation Therapist will provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Responsibilities include conducting evaluations, reassessments, and treatments, while working collaboratively with an interdisciplinary team to develop and implement individualized treatment plans.

Responsibilities

• Provide clinical rehabilitation services according to licensure and scope of practice.
• Develop, implement, and carry out individualized treatment plans in collaboration with the supervising therapist and physician.
• Utilize evidence-based therapeutic interventions to maximize residents' function, independence, and quality of life.
• Educate residents, families, and caregivers on treatment plans and progress.
• Complete accurate and compliant documentation of patient care activities.
• Supervise therapy assistants as applicable.
• Be available to work on Hawaii State and Federal Holidays as necessary based on facility operations.

Required Experience / Certifications / Licensure

• A minimum of two (2) years of experience in a Skilled Nursing Facility or Intermediate Care Facility setting preferred.
• Current State of Hawaii license in good standing appropriate to the discipline.
• Current Basic Life Support (BLS) certification.
