This role ensures residents are supported emotionally, socially, and in planning for successful transitions of care. The ideal candidate will be compassionate, highly organized, and experienced in long-term care or healthcare social work.This position plays a vital role in ensuring resident rights, dignity, and quality of life are maintained throughout their stay at Oakview Nursing and Rehabilitation.Key ResponsibilitiesComplete comprehensive psychosocial assessments for new admissions and ongoing resident evaluationsDevelop, implement, and update individualized social service care plansParticipate in interdisciplinary care plan meetings and resident conferencesCoordinate admissions, discharges, and safe transition planning to home or other levels of careAssist residents and families with adjustment to placement, coping strategies, and emotional supportServe as a liaison between residents, families, physicians, and external agenciesEnsure compliance with all federal and state regulations related to long-term care social servicesMaintain accurate, timely, and thorough documentation in resident recordsAssist with advance directives, guardianship arrangements, and resident rights educationSupport grievance resolution and advocate for resident needs and preferencesCoordinate community resources, financial assistance programs, and discharge planning servicesParticipate in surveys, audits, and quality improvement initiatives as neededMaintain confidentiality in accordance with HIPAA regulationsQualificationsPrevious experience in Social Services, Human Services, Healthcare, or Long-Term Care strongly preferredKnowledge of Medicare/Medicaid regulations and long-term care requirements preferredStrong communication, documentation, and organizational skillsAbility to work effectively with residents, families, and interdisciplinary teamsHigh level of empathy, professionalism, and problem-solving abilityAbility to manage multiple priorities in a fast-paced healthcare environmentExperience with discharge planning and care coordination strongly preferredSchedule & SettingFull-time positionPrimarily weekday schedule (may require flexibility based on resident needs, admissions, or discharge planning requirements)Skilled nursing and rehabilitation facility environmentWe are an Equal Opportunity Employer. #J-18808-Ljbffr Oakview Nursing and Rehab
