Responsibilities Ensure that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team. Assess hospice care patients and families to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status. Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care. Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted‑living facilities, etc.). Provide appropriate support at the time of death and perform bereavement assessment. Qualifications Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing. Current state license as a Registered Nurse (RN). One year of experience in a clinical care setting. Experience in a hospice or home health environment is preferred. 50% travel required. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Valid driver’s license and insurance and reliable transportation to perform job tasks.
