RN Hospital Care Coordinator

Intermountain Health seeks an RN Care Coordinator to deliver patient-centered care coordination in a collaborative, mission-driven environment. In this role, you’ll complete nursing assessments, create and manage individualized care plans, support safe transitions of care, and partner closely with physicians, social workers, and other team members. You will educate patients and families, manage chronic conditions, and help remove barriers to care. Success in this position requires strong clinical judgment, communication, and EHR skills, along with a commitment to quality, safety, and continuous improvement.
