Home Health Licensed Vocational Nurse (LVN)

Our Home Health Licensed Vocational Nurses enjoy some notable benefits:

Specifics on this case:

• Patient requires G-tube feedings and medication management. 
• 8-16-hour shifts.  8am-4pm and 4pm-midnight 
• This position is in Le Grand, about an hour south of Modesto, The shifts available would be
  • every other Sat- 4p-midnight, Sunday 8a-4p
    • sometimes you might be asked to cover a double:16 hour shift 8a-midnight)
    • on the week you work the weekend you would be off the Monday,
  • the week you are off the week end you would cover Monday 4p-midnight
  • and a set weds and thus 4p-midnight
    • you could be asked to cover other shifts as back up, 
    • as well as do 16 hour shifts for coverage if needed. 

As a Home Health Licensed Vocational Nurse, here’s a big-picture view of what you’ll do:

• Provide in house care to patients with illnesses, injuries and chronic diseases, under the supervision of an RN
• Work with a team family, other LVNs, and the RN supervisor to create the best care environment for the patient
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques  
• Educate family on the patient’s plan of care, medication and home safety  
• Collaborate with interdisciplinary team to coordinate patient care

A few must-haves for Home Health Licensed Vocational Nurses:

• Graduate of an accredited vocational nursing program and an active LVN license in California
• Preference of 1 year of LVN experience, ideally in home healthcare.
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing, strong communication skills and compassion
• Pediatric experience preferred
