Description

• Conduct comprehensive evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment strategies
• Implement physical therapy interventions targeting mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and rehabilitation experts to enhance patient outcomes
• Instruct patients and their families on therapy objectives, discharge procedures, and at-home exercises
• Accurately document evaluations, treatments, and patient progress in line with facility and regulatory protocols
• Engage in interdisciplinary care team discussions and case rounds to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous, but not essential

Skills

• Prior experience in a similar role is highly desirable
