Cardiac Case Manager & Discharge Planner No Weekends

Oklahoma Heart Hospital, L.L.C.

Oklahoma Heart Hospital (OHH) in Oklahoma City seeks an experienced Case Manager/Discharge Planner to coordinate discharge planning and care for patient populations. Lead a multidisciplinary team to optimize outcomes and ensure satisfaction for patients and physicians.

Requirements include an RN license in Oklahoma, 3+ years of hospital-based nursing, and willingness to pursue Case Management certification. Weekend hours are flexible and additional hours may be available.
