Individual is required to work 5, 8-hour scheduled shifts per week. Individual should have a minimum of 1 year of experience in a hospital medical records department, preferred. or Medical Records Administrative Course/Medical Record Clerk Shift: 8:00am - 4:30pm Job Description: Under the supervision of the Director of Health Information Management (HIM), the HIM Document Imaging Specialist will complete the scanning process into the Enterprise Medical Record (EMR), enter deficiencies into the Medical Record Deficiency System and analyze the Medical Record for completeness. Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act. (Gov’t. Should have at least one (1) year experience in a hospital Medical Records Department or Certification from a Medical Records Administrative course or Certificate as a Medical Record Clerk. Scanning knowledge and experience preferred. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Effective oral and written English language communication and interaction skills. Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Effective time management skills to permit working in a fast-paced, results-oriented environment. Orientation to detail. To perform this job successfully, an individual should type 40-50 wpm. Computer terminal experience and basic working knowledge of commonly used business software (including but not limited to Microsoft Office, Word, Excel and e-mail). LICENSURE/CERTIFICATIONS: To perform this job successfully, an individual should be a high school graduate or equivalent. The employee must maintain a valid CA driver's license and current DMV report, the results of which are within acceptable standards, as per SVMC policy as a condition of employment. The employee will also be required as a condition of employment to enroll in the DMV Pull Notice program to allow the Hospital to receive notice of any changes in their driving record. The Employee must maintain at least state required limits of liability insurance/evidence of financial responsibility on my vehicles (such limits may not be adequate and I should consider higher limits for my own protection). Responsibilities and Essential Functions: Prepares medical records for scanning, does quality review, organizes medical records in a timely manner. Analyzes medical records of discharged patients for completeness and accuracy according to Joint Commission guidelines, department and hospital policy and procedures. Notes any deficiencies and refers to appropriate area for follow up. Completes scanning and indexing within established timeframes. Scans and indexes health and administrative information into the Hospital’s Electronic Health Record System. Scans and indexes loose medical record documents received daily. Conducts quality audits to ensure the integrity of scanned images into the Electronic Health Record System. Interacts closely with the HIM Lead and/or Manager when discrepancies occur involving the computer system which affects the Electronic Health Record System functions. Chart pick up for both internal and external departments, which may include, but is not limited to RHC, Wound Care, Urology, ASD, MOB, etc. This will require driving to the locations. Re-indexes and/or relinks documents as indicated within the Electronic Medical Record System.
