Outpatient Physical Therapist II - Schaumburg - Full-Time

Job Summary Hourly Pay Range: $38.67 - $59.94 – The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Sign on Bonus Available! Physical Therapist II – Rehab Services (Days/Evenings) Location: Schaumburg, IL Full time/Part time: Full time, 40 hours per week Hours: Days with 2 evening shifts until 7:30pm required. Required Travel: No Responsibilities Provide 1:1 Care – No double booking. Demonstrate the knowledge and skills necessary to assess, treat, and care for patients in an assigned area. Implement assessment, treatment, and care based on the developmental stages of the patient. Interpret verbal and nonverbal communication of the patient. Evaluate patient’s functional abilities/disabilities utilizing techniques appropriate to the patient type, including assessment of strength, range of motion, pain, sensation, balance, transfers, ambulation status, wound size and condition. In accordance with patient evaluations, establish a treatment plan that specifies treatment goals and modalities to assist the patient in regaining function, increasing strength, and decreasing pain. Administer and instruct patients in the performance of all aspects of the treatment plan, assessing effectiveness and modifying as necessary. Prepare and complete all patient records, billing, and documentation according to departmental guidelines. Communicate and cooperate with physicians, nursing, and other staff to provide optimal patient care. Involve the patient, family, and caregivers in the plan of care, including next-level-of-care planning. Conduct patient/family education sessions and provide self-directed programs considering the developmental needs of the patient. Assist with supervision of paraprofessional staff, students, and volunteers as directed; delegate tasks and monitor performance to ensure completion. Utilize correct body mechanics. Adhere to hospital and departmental safety precautions and infection control procedures to maintain a safe patient care and working environment. Assist with planning and conducting in-service programs; participate in staff meetings, rehab conferences, and educational programs. Attend all mandatory in-services. Assume responsibility for own professional growth; keep abreast of developments in the field of Physical Therapy and incorporate approved developments into treatment and evaluation plans. Participate in the departmental Quality Assurance Program and special assignments as requested. Adhere to all Northwest Community Hospital standards, policies, and procedures. Various levels may have additional functions. Qualifications Completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
