Your critical thinking and clinical knowledge will help support improved patient outcomes and enhance healthcare delivery./ppIn this role, you will:/pulliAdminister oral, injectable, intravenous, and topical medications./liliEvaluate complex patient conditions, monitor pain levels, and document treatment responses./liliApply advanced knowledge of vital sign monitoring to identify and record abnormalities in blood pressure, heart rate, respiratory rate, and temperature./liliPerform routine assessments to monitor patient health and ensure overall well-being./liliManage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management./liliConduct medication reviews to identify potential side effects and drug interactions./liliDevelop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function./liliAccurately document patient medical historyincluding illnesses, medications, allergies, surgeries, and family historyusing electronic health records./li/ulpUse your excellent communication skills to interact with patients, families and other healthcare professionals Required Qualifications/pulliActive and in good standing state LVN or LPN license./liliSuccessful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses)./liliKnowledge of medical terminology and clinical documentation standards./liliExperience with phlebotomy, vaccine administration, and point-of-care testing./liliExcellent communication and organizational skills./liliExperience working in a clinical setting with diverse cultural dynamics among staff and patients./liliProven ability to manage sensitive information ethically and responsibly./liliStrong problem-solving, critical thinking, and decision-making skills./liliProficiency with electronic medical record (EMR) systems; Epic experience preferred/li/ulpEducation/pulliHigh school diploma or GED/liliGraduate of a State approved Practical or Vocational nursing program./liliSuccessful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses./li/ulpPay Range/ppThe typical pay range for this role is:/pp$24.64 - $53.53/ppThis pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
