The requirements is for candidates travel 50-75% of the time to meet with members face to face./ppSchedule is Monday-Friday, 8:00am-5:00pm, standard business hours./ppWe are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team. Case Management Coordinator is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the members overall wellness. Case Management Coordinator will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a members overall wellness through integration. Case Management Coordinators will determine appropriate services and supports due to members health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports./ph3Required Qualifications/h3ulliBi-lingual in English and Spanish./lili1 + year of experience in behavioral health or a long-term care setting/liliComputer proficiency in Microsoft Word, Excel, and Outlook/liliMust reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL/liliMust possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face in North Miami FL. Mileage is reimbursed per our company expense reimbursement policy/li/ulh3Preferred Qualifications/h3ulliAbility to multitask, prioritize and effectively adapt to a fast-paced changing environment/liliEffective communication skills, both verbal and written/liliManaged Care experience/liliCase management and discharge planning experience/li/ulh3Education/h3ulliBachelors degree required, preferably in Social Work or related field required/liliMasters degree preferred/li/ulpAnticipated Weekly Hours: 40/ppTime Type: Full time/ppPay Range: The typical pay range for this role is: $21.10 - $40.90. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
