Job Title Care Manager I, Child & Youth (Chattanooga, TN) Schedule & Compensation Mon‑Fri, 8a‑4:30p; Base salary: $41,000; Additional incentive for language skills and behavioral health experience; Mileage reimbursement available. Job Summary Provide comprehensive care management services to children and youth with Severe Emotional Disorders (SED) and/or adults with Severe Persistent Mental Illness (SPMI). Develop relationships with consumers, assess needs, and coordinate care to help them achieve healthy, successful lives in the community. Responsibilities Engage consumers monthly to assess needs, develop care‑plan goals, and coordinate care. Conduct community visits (home, school, court, etc.) to evaluate needs and connect with individuals where they are. Address mental health, physical health, social determinants of health, and gaps in care, including ED/IP utilization. Provide side‑by‑side support to address barriers to engagement in treatment. Assist with referrals, linkages to community resources, and advocacy for consumers. Collaborate with consumers’ support systems and identify or increase supports as needed. Follow training handouts and policies to ensure consumer care quality. Maintain timely and comprehensive documentation per agency policies. Meet productivity standards per policies and procedures. Actively participate in daily team meetings, bringing prepared insights and developing action plans. Follow up on plans developed in meetings. Participate in monthly supervision: discuss caseload, engage in professional development, and receive feedback. Build and maintain collaborative relationships within departments and with community partners. Demonstrate Integrated Care Core Values of connection, innovation, compassion, and integrity. Requirements Bachelor’s Degree in a health‑related field: counseling, psychology, social work, or other behavioral health field. Alternatively, bachelor’s degree in criminal justice with at least fifteen college‑level semester hours of behavioral health coursework. Valid Tennessee Driver’s License.
