Psychiatric Mental Health Nurse Practitioner - Primary Care Network - Saratoga County, New York Compensation Range: $124K - $164K/Yr (tiered based on experience) Join Saratoga Hospital Medical Group - Behavioral Health as a Psychiatric Nurse Practitioner working within our primary care network. The role begins at Saratoga Community Health Center in downtown Saratoga Springs, with an initial orientation period then transitioning to another primary care practice. Candidates must have 1-2 years of outpatient experience and prior psychiatric service experience. Primary Job Responsibilities Assessment: Perform independent age‑appropriate mental health assessments for patients with psychiatric diagnoses, provide crisis response, medication management, direct mental health services, and documentation. Psychiatric Care Planning: Formulate initial diagnosis and treatment plans, monitor intervention effectiveness, provide psychotherapy and supportive counseling, facilitate transitions across health care settings, and educate patients and families. Communication and Collaboration: Communicate thoroughly with all disciplines, collaborate with multidisciplinary team members, make referrals, provide consultations, and educate staff on behavioral health topics. Leadership: Serve as a clinical role model, lead clinical skill development, promote safe practice, and act as liaison/consult on behavioral health issues. Committees/Participation: Participate in applicable committees, continue education through workshops, conferences, in‑services, and self‑study. Personal Evaluation: Conduct self‑evaluation, seek feedback from all disciplines, identify practice changes to improve patient outcomes. All physicians and nurse practitioners take inpatient call for a 16‑bed inpatient adult mental health unit; call is paid and approximately 1:10, taken evenings by phone with rounding on weekends. General Medical APP Responsibilities (portions apply to PMHNP) Obtain, document, and update medical history; identify health risks; perform physical examinations; conduct preventive screening; formulate differential diagnosis and care plan. Order and review diagnostic tests; prescribe and manage pharmacologic therapies. Determine effectiveness of care plan via documentation; reassess and modify plan as needed. Make referrals to other health professionals and community agencies; admit/discharge to appropriate service. Participate in team meetings and conferences to enhance interdisciplinary approach. Serve on departmental and multidisciplinary committees. Develop and participate in quality improvement programs and standards of care. Maintain current knowledge of professional practice; maintain all required documentation (license, certifications, infection control, ACLS/BLS, DEA). Act as a resource to staff and implement educational programs as needed. Minimum Qualifications Licensed as a Nurse Practitioner in the State of New York. Minimum of two years’ experience in a psychiatric or mental health center setting. Must possess a New York State NP license for credentialing at Saratoga Hospital. #J-18808-Ljbffr Capacity Path
