Licensed Practical Nurse

The Licensed Practical Nurse (LPN) is responsible for providing nursing care and administering medications and treatments to residents in accordance with physician orders, community policies, and applicable Illinois regulations. The LPN works collaboratively with the healthcare team to promote resident health, safety, dignity, and quality of life. Essential Duties and Responsibilities Administer prescribed medications, injections, and treatments accurately and safely. Monitor residents for changes in condition and report concerns to the Registered Nurse, physician, or other appropriate healthcare provider. Document medication administration, treatments, and resident observations accurately in the electronic health record (EHR/eMAR). Review physician orders and ensure timely implementation. Coordinate medication refills and communicate with pharmacies and healthcare providers. Perform medication cart audits and maintain medication storage according to state and federal regulations. Assist with resident assessments and care planning. Respond promptly to resident emergencies and provide appropriate nursing interventions. Educate residents, families, and staff regarding medications and health concerns as appropriate. Ensure compliance with infection prevention practices, resident rights, and safety standards. Participate in quality assurance and regulatory compliance activities. Collaborate with caregivers and other interdisciplinary team members to provide quality resident care. Qualifications Current Illinois Licensed Practical Nurse (LPN) license in good standing. Current CPR/BLS certification. Previous experience in assisted living, long-term care, memory care, or skilled nursing preferred. Knowledge of medication administration and nursing documentation. Strong communication, organizational, and time management skills. Ability to maintain confidentiality and work effectively with residents, families, and team members. Physical Requirements Ability to stand, walk, bend, and reach throughout the shift. Ability to lift, transfer, and reposition residents in accordance with community policies.
