Reporting to the Program Specialist, the CCP promotes the community support of people with developmental and intellectual disabilities by providing direct care and training of individuals in a program setting.Key Responsibilities:Engage individuals according to our mission.Utilize programmatic resources.Monitor the implementation of high quality individual support plans.Promote and follow Delta’s Risk Management Program for the health and safety of individuals and employees.Maintain all required training per Delta policy and state regulations.Provide for the care, safety and well-being of individuals who attend the Community Center, participate in Community Participation Supports in the community, and receive In Home Supports.Work cooperatively with other Community Support Professionals in the implementation of various activities for groups of individuals.Conduct and write annual interest/skill assessments for assigned individuals.Participate as a team member in the individualized program ISP/ICP process for assigned individuals.Implement ISP/ICP outcomes and other specified supports for designated individuals.Complete routinely required paperwork in a timely and accurate manner.Actively participate in routinely scheduled meetings and trainings.Communicate continuously with your Supervisor.We believe in rewarding experience and professional development.
