Ensure safety principles are consistently applied and collaborate closely with physicians, RN case managers, and other healthcare professionals to optimize patient outcomes.Key Responsibilities:Conduct hospice admission assessments and home health admission assessments (OASIS), developing initial patient care plans.Coordinate with patients, families, and interdisciplinary teams to address care needs and update plans accordingly.Supervise Home Health Aide services in accordance with organizational policies.Participate in on-call rotations to provide 24-hour nursing support for hospice, homecare, and private duty patients, including scheduled visits and admissions.Maintain accurate and timely documentation following agency guidelines.Foster positive relationships with patients, families, colleagues, physicians, and referral sources.Encourage patient independence by setting care goals and involving patients and families in discussions about medical issues, medications, testing, and self-care skills.Observe and assess patients' physical, mental, emotional, and spiritual conditions, collaborating with team members as needed.Adhere strictly to HIPAA regulations to protect patient health information.Education:Graduate of an accredited nursing program.BSN strongly preferred.Experience:Several years of clinical nursing experience.Previous hospice and/or home health experience preferred.Certification as a Hospice and Palliative Care Nurse is a plus.Requirements:Current Registered Nurse licensure in the state of Kansas.BLS certification required within three months of hire.Valid Kansas driver's license and auto insurance meeting state requirements.Proficiency with MS Office and strong keyboarding skills.Excellent communication, multitasking, and prioritization abilities.Behavioral Skills:Integrity: Demonstrates honesty and ethical behavior, inspiring others.Compassion: Shows empathy and delivers high-quality customer service.Accountability: Takes ownership of responsibilities and adapts to changing priorities.Respect: Collaborates effectively with diverse populations and maintains a positive service orientation.Excellence: Strong leadership, communication, and strategic thinking skills with the ability to manage complex projects and facilitate process improvements.This role serves patients and families in the Midwest region, providing essential hospice and homecare nursing services with a warm, patient-centered approach.
