Responsibilities:

• Collaborate : Work closely with our interdisciplinary care team, including nurses, therapists, physicians, and other healthcare professionals to develop and implement individualized care plans.
• Coordinate : Collect and analyze medical information, submitting accurate and complete Minimum Data Set (MDS) assessments and care plans to Medicare and other regulatory agencies.
• Communicate : Effectively communicate with care team members, families, and healthcare providers to ensure seamless transitions and optimal care outcomes.
• Monitor : Monitor and report changes in resident conditions, providing timely and accurate information to the care team and families.
• Maintain : Maintain accurate and confidential records of resident care, assessments, and care plans.

Requirements:

• Licensure : Current RN license in the state of Colorado
• Education : Bachelor's degree in Nursing or related field
