Job Responsibilities

• Conduct evaluations for patients in inpatient and outpatient settings to develop personalized treatment strategies
• Deliver targeted physical therapy interventions that enhance mobility, strength, balance, and overall function
• Partner with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge plans, and at-home exercises
• Accurately document evaluations, treatments, and progress in line with facility and regulatory standards
• Engage in care team rounds and interdisciplinary conferences to ensure cohesive patient care

Education and Certifications

• Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized institution
• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications in specialized areas (neurological, geriatric) are beneficial, though not mandatory

Skills

• Prior experience is strongly preferred
