Care Transition Specialist – Case Management Work location: Texas Health Allen, 1105 Central Expwy North, Allen, TX 75013 Work hours: Part time; 3 days/week – 8:30 – 5:00p Care Management Highlights Engaged leadership team Manageable case load Team that works together to ensure work/life balance for all Here’s What You Need H.S.
