Licensed Practical Nurse – Long-Term CareFull Time D/N rotation (5x8s), EOW and Holiday required.None provided.Current LPN Licensure in New York, BLS certification (through AHA or RQI), 1 year experience as an LPN.Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team members. Contribute to the development of age-appropriate and culturally relevant care plans that reflect nursing diagnoses and interventions. Recommend revisions to care plans based on evaluations of resident outcomes and document referrals as necessary. Maintain knowledge and competency in nursing practices, participating in mentoring and orientation of new staff.
