How You'll Work You’ll provide home-based skilled speech therapy care for patients as directed by the attending physician, educate patients and their families, and ensure patient safety. Major Areas of Responsibility Plan of Care: Develop an individualized ST plan of care and make adjustments as needs change. Guide the patient in use of therapeutic, creative and self-care activities to improve functioning. Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff. Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or other speech therapy related functions. Help decrease re-hospitalizations by teaching preventive measures and promoting good decision-making. Act as a Family Educator/Advocate, counseling family on the disease/injury and how to manage in all phases of speech therapy. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Understand and follow agency policies, procedures, rules, and regulations, and communicate changes in schedule/availability to schedulers or supervisors. Attend in-service training and mandatory agency meetings. Hard & Soft Skills Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians, and coworkers. Attention to detail and observant follow-through. Requirements Speech Therapist with a current license in the state of employment. Minimum one-year experience as a ST in an acute care setting. Home care experience preferred. Valid driver's license and auto insurance in your name as a driver. Capable of all physical demands of the role.
