Core Responsibilities
- Conduct comprehensive nursing assessments for senior patients in home and community-based settings across assigned Michigan territories
- Develop, implement, and regularly update individualized care plans in collaboration with physicians, therapists, and other interdisciplinary team members
- Administer medications, wound care, IV therapy, and other skilled nursing interventions as ordered by supervising physicians
- Monitor patient health status and document clinical observations accurately using electronic health record systems
- Educate patients and family caregivers on disease management, medication adherence, fall prevention, and overall wellness strategies
- Coordinate transitions of care between hospital, rehabilitation, and home settings to ensure continuity and patient safety
- Communicate proactively with physicians regarding changes in patient condition and advocate effectively for patient needs
- Maintain full compliance with all state and federal home health regulations, HIPAA requirements, and internal clinical policies
- Participate actively in quality improvement initiatives, staff meetings, and ongoing professional development programs
Must-Have Requirements
- Current, unrestricted Registered Nurse license issued by the State of Michigan
- Minimum of two years of clinical nursing experience, preferably in home health, geriatrics, or a related specialty area
- Strong working knowledge of chronic disease management, including conditions commonly affecting elderly populations such as diabetes, heart failure, COPD, and dementia
- Reliable personal transportation and valid Michigan driver's license with ability to travel to patient homes throughout the assigned service area
- Proficiency with electronic health record documentation systems and standard healthcare technology tools
- Excellent communication, critical thinking, and time management skills essential for independent practice
- Current BLS certification required; additional certifications such as OASIS training considered a strong asset
