Social Services Director:

Vivo Healthcare Winter Haven is looking for an experienced Social Services Director to lead and manage social services programs and staff, ensure compliance with regulation and effectively coordinate services for individuals and the community. Director is also responsible for facilitating discharge planning for residents on their journey back home.

Qualifications:

·         High School Diploma or equivalent required

·         Licensed Social Worker preferred. 

·         LTC/SNF experience preferred
