Description

• Conduct comprehensive assessments of patients in both inpatient and outpatient environments and craft personalized treatment plans
• Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Engage collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to ensure the best outcomes for patients
• Educate patients and their families about therapeutic objectives, discharge plans, and recommended home exercises
• Maintain thorough documentation of evaluations, treatments, and patient progress aligned with facility standards and regulations
• Participate actively in care team discussions and interdisciplinary meetings to ensure continuity of care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• Preferred: CPR certification
• Additional specialized certifications (e.g., neurological, geriatric) are advantages but not mandatory

Skills

• Prior experience in a clinical setting is highly preferred
