Lead Clinical Excellence and Help Shape the Future of Ambulatory Pharmacy CareWe are seeking an experienced Ambulatory Team Leader Pharmacist to provide clinical leadership, operational oversight, and advanced pharmaceutical care within our ambulatory pharmacy services. In this role, you will combine direct patient care responsibilities with leadership and mentorship opportunities, helping ensure high-quality, patient-centered care while supporting the growth and development of pharmacy team members.As a clinical leader, you will work closely with physicians, advanced practice providers, pharmacists, technicians, and other healthcare professionals to optimize medication therapy, improve patient outcomes, and advance ambulatory pharmacy services. This position is ideal for a pharmacist who enjoys leading teams, improving processes, mentoring others, and practicing at the top of their license.What You'll DoProvide comprehensive medication management and advanced clinical pharmacy services for ambulatory patientsDesign, monitor, evaluate, and optimize patient-specific pharmacotherapy plansCollaborate with physicians and interdisciplinary care teams to improve patient outcomesProvide patient education and medication counselingSupport services such as anticoagulation management, medication reconciliation, pharmacokinetic dosing, and other ambulatory pharmacy programsRespond to medication-related questions and provide drug information to patients and healthcare providersParticipate in quality improvement initiatives and identify opportunities to enhance patient care, operational performance, and cost savingsServe as a clinical resource and mentor for pharmacists, technicians, and Medical Office AssistantsProvide day-to-day leadership, oversight, and training to pharmacy team membersAssist with onboarding, orientation, and ongoing professional development activitiesPrecept pharmacy students and residents as appropriateContribute to educational programs through presentations, in-services, newsletters, and community outreach initiativesSupport compliance with all regulatory, accreditation, and professional practice standardsContribute to departmental financial performance, operational effectiveness, and strategic growth initiativesMaintain pharmacy systems, records, formularies, and medication-use processes in accordance with organizational standardsWhat We're Looking ForBachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacyLicensed or eligible for licensure in the applicable state(s) of practiceAbility to obtain required licensure within 45 days of employmentHospital pharmacy experience requiredAdvanced pharmacy certification required (such as BCPS, BCACP, BCOP, CACP, MBA, or other relevant advanced credential)Strong clinical knowledge and commitment to evidence-based patient careExcellent communication, leadership, and relationship-building skillsAbility to collaborate effectively with interdisciplinary healthcare teamsStrong problem-solving and decision-making abilitiesCommitment to mentoring, staff development, and continuous improvementPreferred:Previous leadership or direct supervisory experienceExperience leading ambulatory care, clinical pharmacy, or medication management programsExperience with quality improvement and performance management initiativesWhat We OfferA leadership role that combines advanced clinical practice with team development and operational impactThe opportunity to influence the growth and future direction of ambulatory pharmacy servicesMeaningful collaboration with physicians, providers, and interdisciplinary healthcare teamsOpportunities to mentor pharmacists, technicians, students, and residentsProfessional growth through leadership responsibilities, quality initiatives, and strategic projectsA highly collaborative environment where pharmacists practice at the top of their licenseThe chance to directly improve patient outcomes through comprehensive medication managementA supportive culture focused on innovation, learning, teamwork, and clinical excellenceA stable healthcare organization committed to advancing patient care and community healthJob SummaryRequisition Number: TEAML017767Job Category: PharmacySchedule: Full-Time
