Field-Based Health Home Care Manager (NYC)

FreedomCare is seeking a Health Home Care Manager to join our Care Management team in New York. This field-based role requires frequent travel to patients' homes in Manhattan and Bronx. The ideal candidate will possess an Associate's degree and at least 2 years of Care Management experience, demonstrating strong communication and assessment skills. As a Health Home Care Manager, you will engage with patients, coordinate care plans, and provide direct service to a caseload of chronically ill individuals, helping them achieve their health goals through a supportive and compassionate approach. #J-18808-Ljbffr
