Pay: $55.00 - $65.00/hour Weekend and after-hours on-call rotations Comprehensive benefits package including medical, dental, vision, retirement benefits, and paid time off Complete patient assessments and determine home care needs Develop and revise individualized plans of care Administer medications and treatments as ordered by physicians Educate patients and families regarding treatment plans and healthcare needs Coordinate care with physicians and interdisciplinary healthcare teams RN Case Manager Qualifications: Minimum of one year of recent acute care experience required Current BLS/CPR certification required Valid driver's license and auto insurance required Home health experience preferred The Surrounding California Community: Serves patients throughout Carmel Valley, San Marcos, Carlsbad, and surrounding communities Minutes from beautiful Southern California beaches and outdoor recreation Excellent year-round climate with abundant sunshine Family-friendly communities with top-rated schools, shopping, and dining #J-18808-Ljbffr Ironside Human Resources
