Paid time off (vacation, holiday & sick days). About the job: Schedule: Monday–Friday 8:00 AM–5:00 PM. Salary: $70,000–$80,000 annually. The Social Worker‑MSW, under the supervision of the Center Manager, plans, organizes and implements social services to Senior Community Care participants and families. Responsibilities include assessment, treatment, teaching and counseling. Responsibilities include, but are not limited to: Conduct assessment and ongoing case management. Provide individual participant contacts and collateral contacts. Offer participant and family education, assessment, counseling and provision of resources. Advocate and ensure participant and caregiver needs are met. Assist with disenrollment procedures. Liaison between IDT, caregiver representatives, and community agencies. Conduct in person initial assessments for enrollment to obtain a complete psychosocial history. Coordinate with Interdisciplinary Team to develop comprehensive plan of care. Conduct re‑assessments every six months and as needed. Maintain regular attendance at Interdisciplinary Team meetings, communicate participant changes. Provide ongoing support, counsel, and education on aging, dementia, grief, end‑of‑life, disease processes, etc. Present requests to IDT for admission/discharge to contracted facilities. Facilitate meetings with participants, families, caregivers, community agencies; mediate discussions. If hospice care is appropriate, provide emotional support, education, funeral/financial planning referrals, and facilitate hospice or nursing home placement. Initiate referrals to external resources such as Adult Protective Services, Housing Authority, public utility companies. Advocate with entities for community stability. Required Qualifications Master’s degree from an accredited school of social work (required). Licensed, registered or certified in the state of employment (required). Minimum of one year’s experience working with frail or elderly population (required). Experience working on a multi‑disciplinary team in a hospital, nursing home or community‑based setting (preferred). Must clear background check. Must have a valid driver’s license and means of transportation. Must have medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
