Senior Care Coordination Social Worker

Wellstar is seeking a Care Coordination Social Worker Sr to assess psychosocial needs and orchestrate patient discharge plans. You will coordinate with the care team, guide families to community resources, and ensure a smooth transition across settings. The role includes mentoring staff and participating in interdisciplinary rounds to optimize patient outcomes.
