Community Liaison

Caring Hearts Hospice Inc. | Edmond, OK | Full-Time About the Role Caring Hearts Hospice Inc. is seeking a mission-driven Community Liaison to build referral relationships and connect patients and families with compassionate, patient-centered end-of-life care. This role plays a key part in supporting our organization's growth by promoting our services throughout the community, developing strong partnerships with healthcare providers, and helping ensure that more families have access to the hospice care they need, all while supporting compliance with Federal, state, and accreditation requirements, including Medicare and Medicaid regulations. This position reports directly to the Community Liaison Director . What You'll Do Analyze the company's service area and marketing data to identify target markets, strengths, weaknesses, and opportunities Visit physician offices, hospitals, assisted living facilities, skilled nursing facilities, and other referral sources to present agency credentials and generate patient referrals Develop and implement a structured monthly marketing plan and outreach calendar Conduct professional, mission-aligned outreach with referral sources and community partners, maintaining clear communication with internal teams and leadership Attend and actively participate in IDG, coordination, and leadership meetings Support office staff as needed, including obtaining physician signatures and delivering IDG updates Provide community outreach and education on hospice services Manage a monthly operational budget for outreach and marketing efforts Work toward a monthly new-patient admission goal in support of census growth, with team support along the way Requirements Valid driver's license with an insured automobile in good working order Ability to travel within a 60-mile radius of Edmond, OK for visits to referral sources, patient homes, and facilities Strong interpersonal, communication, and relationship-building skills Comfort working toward outreach and admission-based goals in a healthcare setting Physical Requirements & Work Environment Involves sitting, standing, walking, stooping, and phone-based communication Minimal lifting of office records, files, and printed materials Regular local driving required as part of daily duties Schedule Monday through Friday, full-time Monthly and quarterly admission-based bonus opportunities Mileage reimbursement for approved work-related travel Medical, dental, and vision insurance Life insurance coverage IRA retirement plan with employer match Paid Time Off (PTO) Paid holidays Caring Hearts Hospice Inc. is an organization dedicated to supporting the physical, emotional, and clinical needs of patients and families during end-of-life care.
