Nurse Practitioner Internal Medicine

Job Description Great new career opportunity for Internal Medicine and/or General Practice Nurse Practitioners in Western Massachusetts. Salary Range: $125,000 to $135,000 a year. The Advanced Practice Nurse (NP) will be responsible for, but not limited to: Conduct visits to patients on palliative care in accordance with local, state, and federal regulations. Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care. Facilitate access to appropriate support models of care (hospice, rehabilitation, ongoing aggressive treatment). Consult with other members of the interdisciplinary team to contribute to the patient’s plan of care. Prepare and maintain accurate patient records, charts, and documents. Assist in development of clinical practice guidelines/standards to support quality palliative care as needed. Ensure collaboration with patients, caregivers, medical directors, and members of the IDT. Education Master's Degree in Nursing. Professional Experience Must have a Nurse Practitioner’s license in the state of operations. Must have at minimum one year of experience as a Nurse Practitioner in a hospital, skilled nursing, or acute care setting (or similar field). Board Certified in Internal Medicine or General Practice. Must have a Master’s Degree. Must have a valid driver’s license and reliable transportation. Must be willing to travel locally.
