Home Health Nurse

Provide skilled nursing care and comprehensive patient assessments in the home setting

Develop, implement, and manage individualized care plans

Perform wound care, wound vac management, and central line care

Administer IV therapy and perform venipuncture procedures

Manage Pleurex catheter care and other specialized treatments

Educate patients and families on disease management, medications, and care plans

Coordinate care with physicians, therapists, caregivers, and interdisciplinary teams

Accurately document patient visits and updates within the EMR system

Monitor patient progress and respond effectively to changes in condition

Manage daily schedules efficiently while maintaining quality patient care

Minimum 1 year of Home Health nursing experience required

Prior visiting nurse experience strongly preferred

First-time travelers will not be considered

Wound vac management and wound care

Active Illinois RN License or Compact RN License

American Heart Association BLS Certification

Valid driver's license

Reliable vehicle with active auto insurance

Technology Experience

EMR experience preferred, especially with Epic

Experience with Sapphire IV pumps preferred
