Care Manager

HOURS PER WEEK: 35GENERAL SUMMARY:The Care Manager is responsible for maintaining a caseload of older adult and individuals with disabilities who live in the community. Responsibilities include conducting home visits, assessing consumer needs, determining program eligibility, developing and coordinating service plans, and updating consumer records in a timely manner. Working as part of an interdisciplinary team, the Care Manager collaborates with agency nurses and community nursing facilities.ESSENTIAL FUNCTIONS:Conduct in-home functional assessments of consumers.Develop service plans with the consumer and their support network and monitor their ongoing effectiveness.Arrange services through contracted vendors and other community resources.Assess consumers for risk, abuse and neglect and report such issues as needed.Maintain case documentation in accordance with AGE documentation standards.Assume responsibility for covering Care Manager duties several times per month assisting consumers whose Care Manager is unavailable.Participate in Staff Development including staff meetings and trainings.COMPETENCIES NEEDED:Accountability: Demonstrates a high level of commitment to achieving results. Employees work in varied residential environments based on assigned caseload. Final compensation will depend on the candidate’s experience, skills, and alignment with the role’s responsibilities.QUALIFIED APPLICANTS WILL HAVE:Bachelor’s degree in human services, social work, or related field preferred.
