**Required Experience/Qualifications:**- 1 year Home Health OT experience- Valid driver's license and car insurance- COVID vaccination (declinations not approved)**Strongly Preferred Experience:**- OASIS and start of care experience**Certifications:**- BLS (AHA) certification (must be active at time of submission)**Licensure:**- State license in Illinois (must be active at time of submission)**Additional Submission Requirements:**- Resume- Skills checklist (within 1 year)- 2 supervisory references- COVID card/status (declinations not approved)
