About the Role:
In the role of a physical therapist, the primary focus is on facilitating the restoration of individuals' physical well-being. This involves conducting comprehensive assessments, developing personalized treatment plans that encompass therapeutic exercises and interventions, and collaborating seamlessly with interdisciplinary healthcare teams. Central to the position is the continuous monitoring of patient progress, alongside effective advocacy for their overall health. A proficiency in communication and meticulous documentation is crucial, coupled with a dedication to ongoing professional development for sustained excellence in this role.

Responsibilities:

• Develop and implement physical therapy treatment plans for clients
• Provide physical therapy assessment and evaluations
• Work with patients to improve their range of motion and strength
• Educate and counsel patients and their families on physical therapy treatments
• Document all patient care services and progress
• Maintain patient records
• Conduct thorough assessments of patients' physical conditions and functional abilities.
• Develop individualized treatment plans based on clinical assessments and physician recommendations.
• Implement evidence-based physical therapy interventions to improve mobility, strength, balance, and overall functional independence.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Collaborate with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive and coordinated patient care.
• Provide education to patients and their families on home exercise programs and self-management techniques.

Requirements:

• Licensed Physical Therapist in the state of Florida
• At least 1 year of experience in physical therapy
• At least 1 year of experience working in Home Health environment
• Excellent communication and interpersonal skills
• Strong problem-solving and critical thinking skills
• Ability to lift heavy objects
• Excellent interpersonal and communication skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
