RN Acute Care Transition Specialist

Role: Provides direct patient care in a specialty acute care (Med-Surg) setting, utilizing the nursing process and working within a multidisciplinary team to achieve optimal outcomes. Responsibilities: Prioritizes patient wellness, models proficiency through precepting, contributes to safety and quality at the point of care, and supports evidence-based practices in a fast-paced environment. Qualifications and Skills: Requires RN licensure, BLS certification, at least 6 months of nursing experience, and graduation from an accredited nursing program. Preferred: Bachelor\'s degree in Nursing. Ability to work in day and night shifts, with additional incentives for night shift work.
