Job Description Job Description $10,000 Sign-on Bonus BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full-time opportunity performing home health visits for our Atlantic County Visits office . This office services adult and geriatric clients on a per visit basis in territories throughout Atlantic County. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. Watch this video to learn more about field employee career pathing: Why Choose Home Health? * Build meaningful relationships through one-on-one patient care * Create your own schedule and enjoy greater work-life balance * Focus on quality care without the productivity pressures of a clinic * Work independently while being supported by a collaborative clinical team * Help patients achieve their goals in the environment where they are most comfortable—their home What You'll Do * Perform home visits in designated geographic territories * Complete comprehensive physical therapy evaluations * Assess mobility, strength, balance, gait, transfers, endurance, and safety risks * Develop individualized treatment plans to improve function and independence * Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions * Collaborate with nurses and other clinicians as part of a multidisciplinary care team * Educate patients and caregivers on exercises, safety, and disease management * Document patient care using a user-friendly electronic medical record system Qualifications for a PT - Physical Therapist include: * A current NJ Physical Therapist - PT license. * A minimum of one year of recent work experience as a Physical Therapist - PT. * Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma. * Ability to work independently and manage time effectively. * Strong interpersonal, organizational, and problem solving skills. * Solid computer skills; prior experience with electronic medical records (EMR) preferred.
