Provide primary care services to adult patients through team-based care, with a focus on chronic disease management, routine preventive care, and acute illnesses. Reporting to the clinic administrator, this role collaborates with physicians and other clinicians to support comprehensive, coordinated patient care.

 

Responsibilities

• Manage prescriptions and medication-related care.
• Provide chronic disease management for adult patients.
• Deliver routine preventive care.
• Evaluate and manage acute illnesses.
• Complete accurate clinical documentation.
• Provide follow-up care and monitor patient progress.
• Coordinate referrals and support continuity of care.
• Participate in team-based care with physicians and other clinicians.

 

Preferred Qualifications

• Five or more years of experience in primary care practice.
• Experience with primary care practice.
• Experience managing chronic diseases.
• Proficiency with electronic health records.
• Bi-lingual Preferred
