Crouse’s Care Coordination Services team is hiring a Social Worker . Pay Range: $60,000.00 - $65,905.00 / annual. Available Schedule: Full Time, Day Shift, 8:00am-4:30pm. Social Worker Position Responsibilities Works collaboratively with the Care Managers and other members of the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care. Participates in the development and implementation of a comprehensive integrated discharge plan. Identifies patient and family psychosocial and environmental needs to admission, diagnosis, treatment and discharge. Actively participates in the discharge process following federal and state regulations and accreditation standards managing the care across the continuum to ensure a safe discharge within a timely manner. Provides crisis interventions to patients and families as needed. Social Worker Position Requirements Master’s Degree in Social Work required. LMSW license preferred. 2 to 3 years Social Work required. Case Management, Discharge Planning or home care experience preferred Excellent oral and written communication and interpersonal skills. Demonstrate age appropriate communication in assigned work area. Ability to interact with physicians, members of the multidisciplinary healthcare team, patients and families and community agencies to achieve optimal patient outcomes. Knowledge of federal and state regulations (DOH, Medicare/Medicare). Strong organizational and problem solving skills. Knowledge of third party payers and /or managed care principles.
