About the Role:

As a Resident Care Coordinator, you will be responsible for coordinating and implementing individualized care plans for our residents, collaborating with our interdisciplinary care team to ensure seamless transitions and optimal outcomes. You will also be responsible for:

• Conducting thorough assessments of residents' physical, emotional, and psychosocial needs

• Developing and implementing individualized care plans that meet residents' unique needs

• Collaborating with the care team to ensure timely and effective communication

• Monitoring residents' progress and making adjustments to care plans as needed

Responsibilities:

• Coordinate and implement care plans with the interdisciplinary care team

• Conduct regular assessments and evaluations of residents' needs

• Develop and implement plans to address residents' psychosocial and emotional needs

• Collaborate with families and caregivers to ensure informed and coordinated care

• Participate in Quality Improvement initiatives to improve resident outcomes

Requirements:

• Current LPN/RN license in the state of Illinois

• Excellent assessment, communication, and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Basic computer skills and familiarity with electronic medical records
