Job Description

Take on a vital nursing role providing skilled care to patients in their homes through a full-time, 13-week contract. In this position, youll deliver compassionate, evidence-based nursing services in a field-based setting, utilizing your expertise to foster better patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Expect daily travel within the assigned territory to conduct 3-5 patient visits per day (caseload equates to 30 points per week)

Key Responsibilities:

• Perform comprehensive OASIS assessments, creating individualized care plans.
• Provide direct skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management.
• Closely monitor patient conditions, report changes promptly, and coordinate with physicians and the broader care team.
• Offer patient and family education on diagnoses, medications, treatment plans, and preventive health strategies.
• Maintain detailed, timely records using Home Care Home Base (HCHB) electronic documentation.
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for comprehensive care planning.
• Attend case conferences, staff meetings, and participate in ongoing professional development.
• Uphold all regulatory and agency standards in every aspect of care and documentation.

Required Qualifications:

• Active and unencumbered RN license for the state of practice
• At least 1 year of recent home health experience (field-based strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation is mandatory
• Reliable transportation, valid drivers license, and current auto insurance

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
