LPN - Licenced Practical Nurse Monday - Friday 3pm-11pm

Job Description

We are seeking a Compassionate, experienced, LPN to join our unique, family-owned Memory Care, Assisted Living team! You will be responsible for the assessment, diagnosis, and treatment of residents. This position reports to the director of Nursing.

Responsibilities:

Administer nursing care to residents.
Diagnose and establish patient treatment plans
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Educate resident families about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate residents' medical records
Provide advice and emotional support to patients and their family members

Qualifications:

Previous experience in nursing or other medical fields
Familiarity with medical software and equipment
Ability to build rapport with residents and families
Strong problem solving and critical thinking skills
Day to day operations, scheduling, managing LPN and CNA on shift.
